Click the "University Park Form " link below.

You may open the document with Microsoft Word or save the document to your computer.

Complete the form in the spaces provided. As you type in the boxes, they will expand to fit your content.

After your form is complete, save it and attach the form to an email addressed to Yvonne Gaudelius (ymg100@psu.edu). Place in the "Subject" line " FYE Plan."

This question will display: Does your campus’s First-Year Engagement Plan require a First-Year Seminar of all students?

If your answer is "YES" you will complete FORM A; If your answer is "NO" you will complete FORM B. Links to these forms are at the bottom of the initial page.

When you open FORM A or FORM B, you may open the document with Microsoft Word or save the document to your computer.
