How to Submit a Petition to Faculty Rights and Responsibilities Committee

All petitions to the Faculty Rights and Responsibilities Committee are submitted electronically using secure folders. Each petition must be submitted as one single PDF file.

INSTRUCTIONS
1. Complete this Petition Form. Print it as a PDF file.
2. Combine this form (with form first) with your supporting documentation into one single PDF file.
3. Title your PDF file using the following format: your last name, first name, the word Petition, underscore, and the date (LastNameFirstNamePetition_mmddyy; Example: SmithAnnaPetition_051122)
4. Contact Erin Eckley, Administrative Assistant for the Faculty Rights and Responsibilities Committee (ele2@psu.edu, 814-863-1204) to have a secure folder set up to which you can upload your petition.
5. Upload your petition as one single PDF file to the designated secure folder.
6. Notify Erin Eckley once you have completed the upload.
